All medications, whether prescription or over-the-counter, may be administered only in accordance with the guidelines set forth by the principal of each school. All medications must be taken by parent or guardian to the school office immediately upon arrival at school and must be in original pharmaceutical containers, clearly labeled as to the name of the student, the name of the mediation, the appropriate dosage, and the times of dosage. Any student possessing prescription or over-the-counter medication not in accordance with these guidelines will be considered in violation of the School District’s drug policy and shall be subject to the discipline set forth in the student code of conduct and/or the student/parent handbook.
Parents must complete the proper forms filled out with parent and doctor signatures in order for a medicine to be given to your child at school. The medication must be in a bottle that is properly labeled with:
Student's name
Prescribing physician
Correct dosage
Pharmacy
Expiration date
Students may not transport controlled substances to and from school. Parents must bring those medications to and from school. Medications must be PICKED UP BY PARENT/GUARDIAN. Any medication not picked up from the school by the last school day of the year will be considered abandoned. Abandoned medication will be properly discarded in accordance with local, state, and federal laws/rules by the school nurse and an administrator.
LCSS does not stock over-the-counter medication. All over-the-counter medications must be provided by the parent/guardian and clearly labeled with the child's name.
When a student is identified as having a medical condition requiring medical treatment and consideration an individual health care plan is developed with the school nurse, the student’s physician and the family.
The parent or guardian of a student with a chronic medical condition is required to provide the school with a Health Care Plan as soon as possible and have it updated yearly. Repeated failure of a parent or guardian to provide the school with a Health Care Plan and necessary medications may be considered neglectful and such neglect may be reported to the proper authorities.

Allergic reactions
As a safety precaution, students with chronic medical conditions should provide a copy of the Health Care Plan to the school principal or school nurse or health aide.
In Georgia, the law states that all students who attend school must have a correct and up-to-date immunization form in their file at school. If you do not want your child immunized based on your religious beliefs, you must provide a signed, notarized affidavit stating the reason. A parent or school that allows a student to attend more than 30 days without a correct immunization form or religious exemption is guilty of a misdemeanor and may be required to pay a fine.

7th Grade Immunization Requirements
In Georgia, all children attending 7th grade and new entrants into a Georgia school in grades 8 through 12 must receive one dose of the Tdap vaccine and one dose of meningococcal conjugate vaccine to meet immunization requirements.
11th Grade Immunization Requirements
In Georgia, all students entering or transferring into 11th grade need proof of a meningococcal booster shot (MCV4) unless their first dose was received on or after their 16th birthday. Meningococcal disease is a serious bacterial illness that affects the brain and the spinal cord. Meningitis can cause shock, coma, and death within hours of the first symptoms. To help protect your children and others from meningitis, Georgia law requires students to be vaccinated against this disease unless the child has an exemption.
For a child to attend childcare facility or school in GA a parent can choose one of the following 3 options:
DPH Form 3231 showing a child is vaccinated as required by the state of GA for childcare or school attendance.
Religious Exemption DPH Form 2208 (new form to be used beginning 2015 school year for any new objection/entry to GA school). This form must be submitted as written no alteration allowed. Medical Exemption documented on Form 3231 completed by a physician. The certificate expires annually and needs to be replaced each year upon expiration.
Children whose parents fail to present or renew the Certificate 3231 within the time allotted shall not be permitted to continue in attendance.
